针分离术联合结膜下注射5-Fu治疗青光眼术后早期失败的滤过泡

Needling revision of failed filtering bleb with adjunctive 5-Fu after trabeculectomy

目的 分析针分离术对早期失败的滤过泡的治疗方法、并发症及效果。方法 2 1例 2 1只眼小梁切除术后包裹性囊样滤过泡和扁平性滤过泡的患者行针分离术。结果 针分离术后的眼压明显的低于术前 ,7例(33.33% )仍需用抗青光眼药控制眼压。主要的并发症为角膜上皮毒性反应 ,结膜下出血 ,结膜滤过泡渗漏、脉络膜脱离、低眼压。结论 针分离术联合 5 - Fu可以挽救早期失败的滤过泡 ,增加滤过手术的成功率 ,且并发症少 ,值得临床上推广应用。

Objective To analysis method、complication and effect of needling bleb to treat early failure bleb.Method 21 eyes of 21 cases of encapsulated and flat bleb after trabeculectomy were performed needling revision.Result IOP after needling revision was significant lower than pre-needling revision,7 cases (33.33%) still need anti-glaucoma medicine to control IOP.Major complication:cornea epithelium toxic,subconjunctive hyphema,bleb leak,choroid detachment,hypotony.Conclusion Needling revision with 5-Fu may treat early failure bleb,increase success rate of trabeculectomy,can be widely used in clinic
