Treatment of orbital—maxillary—zygoma complex fracture

目的 详细阐述眶—上颌—颧骨复合骨折的解剖学基础、治疗时机和治疗方案的选择。方法 对 48例眶—上颌—颧骨复合骨折进行分析 ,列举了 4种在治疗中采用的手术方法 :①同时作外眦切开的结膜入路 ;②上唇颊沟口内入路 ;③冠状切口入路 ;④局部进路。并对四种手术进路的方法及其适应症进行详细论述。结果 41例经手术整复的患者经术后观察表明 ,38例骨折复位良好 ,颌面部两侧对称性良好。结论 采取这四种手术进路治疗眶—上颌—颧骨复合骨折 。

Objective: To present the details of orbital—maxillary—zygoma complex fracture(OMZ), treatment timing and the selection of managements. Methods: Forty-eight patients with OMZ complex fracture wer analysed, and four approaches were selected based on the conditions of the fracture: ①transconjunctional approach with lateral canthotomy; ②intraoral approach; ③coronal incision approach; ④local approach. The treatment timing and indications of four approaches were discussed. Results: The results of 38 patients in 41 who got the sursical treatments were satisfactory, and the bilateral symmetry was satisfactory. Conclusion: The four approaches used in orbital—maxillary—zygoma complex fracture can result in satisfactory plastic effect and functional recovery.
