社区高血压病人的药物利用与依从性关系分析

Compliance of Patients with Antihypertensive Therapy at Community in Shanghai

目的 了解社区高血压病人药物利用与依从性状况。方法 利用专门拟定的调查表由调查员入户调查。结果 973名社区高血压病人中有 78.6 %的服用一种药物。常用的五种药物是珍菊降压片 (34.12 % )、硝苯地平 (2 3.12 % )、复方降压片 (2 1.0 7% )、卡托普利 (10 0 7% )与倍他乐克 (5 .5 6 % )。常用药物类型为复方制剂 (5 8.97% )、钙拮抗剂(34.0 5 % )、ACE阻滞剂 (12 .82 % )。高血压病人中依从性佳的比例为 31.1% ,服用一种药物组依从性优于服用二种药物组 (OR =1.72 ,95 %CI 1.13～ 2 .72 )。交感神经阻滞剂的依从性显著优于复方制剂 (OR =2 .84,95 %CI 1.18～ 6 .92 )。结论 :社区高血压病人用药仍以复方制剂为主 ,其有助于改善病人依从性。

Objective\ To survey the use of anti hypertensive and to evaluate the compliance of patients at community. Method\ To interview patients at home with special questionnaire. Results\ There are 78 6% patients taking a kind of medicine in 973 patients. The five drugs of the most generally used,such as Zhengju in 34 12%,nifedipine in 23 12%, combination of drugs in 21 07%, captopril in 10 07%,metoprolol in 5 56%. The groups of the most commonly used medicine are compound preparation of antihypertension(58 97%), calcium antagonsis(34 05%), ACEI(12 82%). The compliance of taking one kinds of medicine group is better than taking two kinds medicine group(OR=1 72, 95%CI 1 13～2 72). The compliance of sympathetic nerve blockers is significantly better than combined antihy pertensive drugs(OR=2 84,95%CI 1 18～6 92). Conclusion:The compound medicine of antihypertension is mainly taken by patients in community, which is helpful to improve the compliance of patients. So some new long effect of compound drug must be developed in future.
