胰十二指肠及肾联合切取的实验研究和临床应用

Experiment Study and Clinical Application of Combined Pancreatoduodenum and Kidneys Harvesting

目的 :探讨同种异体胰十二指肠及肾联合移植尸体供移植物的获取、保存和修剪。方法 :采用原位插管、低温灌注、整块快速切取及体外修剪获得可供移植的胰十二指肠及肾移植物。结果 :8例尸体获取移植物 ,热缺血时间平均为 4min 5 0 s,获取时间为 18min2 0 s,其中 2例用于临床移植 ,胰肾移植物功能均于术后 1周内恢复正常。第 1例受体胰肾有功能存活 2年余。结论 :此方法适合于尸体供胰十二指肠及肾的获取和保存 ,应用于临床效果良好。

Objective:To discuss the harvesting, procurement and preservation techniques of combined pancreatoduodenum and kidneys grafts from cadaveric donors. Methods:Pancreatoduodenum and kidneys grafts were obtained with method of the aortic cannula, perfusion in situ, and harvesting in Ex vivo. Results:The average ischemia time in conventiontal temperature was 4 minutes and 50 seconds in eight cases of donors, but 18 minutes and 20 seconds was needed from brain death to preservation of organs in ice water. The functions of pancreas and kidney grafts returned to normal within 7 days postoperatively. The first patients has been living with grafts′ function for two years. Conclusion:It was suggested that this technique of pancreatoduodenum and kidneys procurement and preparation from nonheart beating donor was reliable, and the grafts were fit for clinical application.
