双功彩色多普勒超声和^(99m)Tc-亚甲基二磷酸盐结合用于乳腺癌诊断的价值 被引量:2

摘要 目的 :评价双功彩色多普勒血流显像 (CDFI)和 99m Tc-亚甲基二磷酸盐 (MDP)结合用于乳腺癌诊断的价值。方法 :用双功彩色多普勒超声检查 15 2例 15 9个乳腺肿块 ,定量分析其彩色象素平均密度、峰值血流速度、阻力指数 (RI)值和血管数目 ;其中 ,32例乳腺肿块用 99m Tc- MDP检测。结果 :乳腺癌彩色象素平均密度为 (6 .10 0± 0 .0 0 2 ) % ,高于良性肿块 (1.5 0 0± 0 .0 0 1) % (P<0 .0 1) ;收缩期峰值血流速度为 (2 3.5 5± 1.2 3) cm/s,明显高于良性肿块的 (5 .73± 0 .39) cm/s(P<0 .0 0 1) ;RI为 0 .71± 0 .0 11,明显高于良性肿块的 0 .5 1± 0 .0 12 (P<0 .0 1) ;血管数目为 1.6 7± 0 .0 6 3,明显高于良性肿块的 0 .86± 0 .0 93(P<0 .0 0 1) ;99m Tc- MDP乳房显像为阳性。CDFI诊断乳腺癌的敏感性 88%、特异性 82 %、准确性 89% ;99m Tc- MDP诊断乳腺癌的敏感性 90 %、特异性 80 %、准确性 87%。结论 :CDFI、脉冲多普勒 (PD)和 99m Tc- MDP联合应用对乳腺癌的诊断有较高的临床价值。 Objective:To evaluate the diagnostic significance of duplex Color Doppler imaging and 99m Tc MDP in breast cancer. Methods:In 152 patients, 158 masses of breast were investigated with duplex Color Doppler imaging and PD. MCPD, PSV, TAV, RI and vessel numbers were analysed quantitatively. 32 of the 158 masses were studied with 99m Tc MDP. Results:MCPD, PSV, RI and vessel numbers in breast cancer seperatively were (6.100±0 002)%,(23.55±1.23) cm/s, 0.71±0.011 and 1.67±0.063, and they were higher than in benign breast group (1.500±0.001)%, (5.73±0.39)cm/s, 0.51±0.012 and 0.86±0.093( P<0.01, P<0.001, P <0 01 and P <0 001) respectively, 99m Tc MDP was positive. Diagnostic sensitivity, specificity and accuracy of CDFI and 99m Tc MDP of breast cancer were 88%, 82%, 89% and 90%, 80%, 87% respectively. Conclusion:CDFI, PD conjugated with 99m Tc MDP may increase the diagnostic sensitivity, specificity and accuracy in clinical diagnosis of breast cancer.
