全胃切除与胃癌根治术中应用器械吻合及缝合600例体会

Experiences in the Application of the Apparatus Anastomosis and Suture on 600 Cases of Total Gastrectomy and Radical Cure for Gastric Carcinoma

自1982年底至今,找院在全胃切除及胃癌根治术中,用器械吻合及缝合共600例,其中全胃切除224例,贲门癌根治术376例。并发症75例,占12.8％;死亡7例,死亡率1.1％;吻合口瘘9例,占1.5％。应用器械吻合及缝合优点:①安全简便;②省时省力,可缩短手术时间1～1.5小时;③吻合口牢靠;④并发症少,吻合瘘比手工吻合减少了7～10倍;⑤术后恢复快,生存率高。本文还详细介绍了使用器械具体操作方法及注意事项,失败教训与成功经验。

From 1982 to 1990, anastomoser and suture were applied in our hospital to 600 cases of gastric cancer and gastric cardiac carcinoma. Of them, 224 cases were of total gastrectomy and 367 cases of radical cure for gastric cardiac carcinoma. Complications were seen in 75 cases (12.8％). Seven patients died (mortality rate: 1.1％). Anastomotic fistula occurred in 9 cases (1.5％). The advantages include: 1) Safety and simplicity; 2) Labour-saving and time-saving and shortening the duration of operation by 1 to 1.5 hours; 3) Firmness of anastomosis; 4) Fewer complications with the incidence of anas tomotic fistula decreased by 10 times compared with manual anastotrosis; 5) Quick recovery after operation and higer rate of survival.
