卡托普利对瓣膜替换术患者心肌心钠素颗粒形态的影响

Effect of Captopril on Atrial Natriuretic Factor granules of Right Auricular Myocytes of Patients with Rheumatic Heart Disease

选择２０例风湿性心瓣膜病伴心功能不全患者，采用随机分组设计，观察口服卡托普利（Ｃａｐｔｏｐｒｉｌ）对血浆心钠素（ＡＮＦ）水平、右心耳ＡＮＦ颗粒及其中、小动脉内径的影响。结果发现，①血浆ＡＮＦ水平降低（Ｐ＜０．０５）；②心肌ＡＮＦ颗粒多于对照组（Ｐ＜０．０１），且以弥漫、团块分布为主；③右心耳中、小动脉内径两组无显著差异（Ｐ＞０．０５）。作者认为，术前准备中服用卡托普利有改善心功能、抑制心肌释放ＡＮＦ的作用。

in order to investigate the captopril on the plasma level of ANF, (Atrial Natriuretic Factor (ANF) granules and inner diameter of artery of the right auricle , 20 patients suffered from rhumatic heart disease before undergoing valve replacement were randomly divided into the test group (n=10) and the control group (n=10) , Captopril was given orally for 2-3 weeks (25mg tid) to the test group before operation. The result indicated that(1) the plasma level of ANF declined in test group (P<0.05, vs. pretreatment level): (2) the number of ANF granules of the control group (P<0.01); (3) the inner diameter of artery between the two groups had no significant difference(p>0.05) .We consider that captopril has the effect of improving the cardiac function and mightinhibit the cells to release ANF. Further research should be continued about the effectof captopril on cardiac blood supply, especially the ventricular muscle before and after cardiac pulmonary bypass.
