摘要 目的 :分析挛缩臀肌的生物力学变化及臀肌挛缩致骨盆倾斜的生物力学机制。方法 :手术中切取的臀肌挛缩带及新鲜病尸的臀肌制成标本 ,用生物力学方法测定其拉伸的力学性质。结果 :臀肌挛缩带拉伸的弹性模量、极限强度和极限应变分别为 (2 .732± 0 .792 )N/mm2 ,(14 8.32± 3 .84)MPa ,0 .896± 0 .315。而正常臀肌的弹性模量、极限强度和极限应变分别为 (0 .14 3± 0 .0 2 4)N/mm2 ,(10 .5 0± 1.6 9)MPa ,1.434±0 .40 2。均有显著差异。结论 :①臀肌挛缩后 ,挛缩带的强度和刚度均显著增大。②臀中肌前部纤维挛缩是导致骨盆倾斜的病因。对有臀中肌挛缩表现的患者 ,一经发现 ,应立即手术 ,以免病变进展 ,造成严重后果。 Objective:To study the biomechanical changes of gluteal muscle contracture and to analyze the biomechanical mechanism of pelvis obliquity caused by it.Methods:Gluteal muscle contracture strips(GMCS) and normal gluteal muscles (NGM) were made into fasciculus.Then the biomechanical characters of these specimens were measured.Results:The elastic modulus,the maximum intension and the maximum strain of GMCS averaged 2.732 N/mm 2,148.32 MPa and 0.896 respectively.In NGM,the elastic modulus maximum intension,and the maximum strain averaged 0.143 N/mm 2,10.50 MPa and 1.434.The difference between elastic modulus and maximum strain of GMCS and NGM was significant ( P <0.05),the difference between maximum intension of GMCS and MGM was significant ( P <0.05),the difference between maximum intension of GMCS and NGM was significant greatly ( P <0.01).Conclusions:(1) The intention and rigidity of GMCS increase more greatly than NGM.(2)The contraction of the anterior part of middle gluteal muscle was the reason of pelvis obliquity.Patients suffering from gluteal muscle contracture should be operated immediately.
