影响脑外伤患者康复效果的相关因素分析

Analysis of factors affecting the rehabilitation effect of patients with brain injury

目的 探讨影响脑外伤患者康复效果的相关因素。方法 10 0例脑外伤患者综合康复治疗后 ,用功能独立性测量 (FIM)评分方法 ,比较FIM改变值 ,采用t检验及多元回归分析等统计学方法 ,探讨年龄、文化程度、损伤类型、手术次数、昏迷时间、康复介入时间、高压氧疗程、神经营养药物、并发症及住院天数与脑外伤患者康复效果的关系。结果 <2 5岁组与 >5 0岁组两组间差异有显著性 (t=2 .0 39,P <0 .0 5 ) ;一次手术与二次及三次手术组比较差异有非常显著性 (t=5 .0 88,6 .42 8,P <0 .0 0 1) ,神经营养药物组与非治疗组比较差异有非常显著性 (t=2 .46 1,P <0 .0 1) ;并发症组与无并发症组比较差异有非常显著性 (t=3.45 0 ,P <0 .0 0 1) ;但多元回归分析显示 ,手术次数与康复效果无关 ;昏迷时间及康复介入时间与康复效果呈负相关 ,高压氧疗程与康复效果呈正相关 ,而文化程度、脑外伤类型及住院天数与康复效果相关性不大。结论 高压氧疗程和脑外伤程度是影响康复效果的主要因素 ,康复介入时间是影响康复效果的次要因素 ,住院天数、手术次数与康复效果无相关性。脑外伤的康复受多种因素的影响 。

Objective To explore the factors affecting the rehabilitation effect in patients with brain injury.Methods 100 cases with brain injury were treated with comprehensive rehabilitation procedures.After treatment,the change values of functional independent measurement(FIM) were observed and compared by FIM Evaluation method.The relation of patient's effect and some factors(such as age,culture,injured type,number of operation,coma duration,time of rehabilitation intervention,courses of HBO,neurotrophic drug,complication and hospitalized period) was detected by t-test and multi-element regression analysis.Results The change values of FIM in patients less than 25 years and those over 50 years presented difference(t=2.039,P<0.05).At the same time,there was significant difference between the first operation and second operation or third operation(t=5.088,6.428,P<0.001);the neurotrophic drug group and non-treatment group(t=2.461,P<0.01);the complication group and non-complication group(t=3.450,P<0.001).Of multi-element regression analysis,the number of operation and effect was no relation.It was found that the effect and coma duration,time of rehabilitation intervention were negative relation,and HBO were positive relation.An addition,the patient's culture,brain injured type and hospitalized period were unimportant.Conclusion It is indictaed that the main factors affecting the rehabilitation effect are the time of HBO and degree of injury,the time of rehabilitation intervention is second.Early comprehensive rehabilitation treatment is key factor for brain injury,because many factors may affect the brain recovery.
