腮腺肿瘤切除后手术野腺泡脱落细胞学的检测意义

The examining significance of acinar cast-off cell on operating region after parotid tumor resection

目的 检测腮腺肿瘤切除后手术野腺泡脱落细胞与复发的关系。方法 2 5例腮腺肿瘤患者 ,各分别取手术野涂片 3张 ,即残存腺细胞 ,冲洗前细胞 ,冲洗后细胞。结果 残存腺泡片检出腺细胞 2 2例 ,其中检出瘤细胞 2例 ,出自包膜完整的多形性腺瘤 ,浅叶和深叶各 1例 ,其原瘤体均在 3cm以上 ;冲洗前后片分别检出腺细胞7例和 1例 ,但均未发现瘤细胞。结论 手术野脱落细胞学检测不受部位和范围的限制 ,它能清楚地观察到瘤旁“正常”腺泡及脱落细胞的状况 ,检出瘤细胞就意味着肿瘤可能复发。

Objective To examine under microscopy acinar cast-off cell on operating region with parotid tumor resection.Methods It was smeared 3 slides each in 25 patients operation region the remains of acinars,the cast-off cell before and after rinsing.Results Under microscopy,it had been found remains of acinus in 22 patients in which 2 cases existed cast-off tumor cell that came from shallow and deep lobe of parotid ploemorphic adenoma that were complete coating and surpass 3 cm in size. It had not been discovered the tumor cell before and after rinsing from remains acinus on operating area.Conclusion The examination of acinar cast-off cell is not limited with the operating area and it can indicate clearly the condition of “normal” acinus by the side of tumor or cast-off cell. If the tumor cell is inspeced from the cast-off cell in operating region,it means the tumor will be recurrent. Of course it has positive pole in prevention tumor recurrent to rinse fully after operation.
