鼻侧腱膜固定法矫正内眦赘皮

Fixation of lateral nasal aponeurosis: a new method for correction of epicanthus

目的 在鼻侧腱膜解剖研究基础上进一步提高内眦赘皮的修复方法。方法 本文对 7具尸体的鼻侧腱膜进行了解剖观察 ,从而提出鼻侧膜固定法矫正内眦赘皮畸形 ,即水平切开半月形赘皮 ,开大内眦角 ,将新内眦点向中央移位 ,并缝合固定于鼻侧腱膜上以重建内眦角 ,修剪新内眦角上下两侧的多余赘皮。结果 解剖发现鼻侧腱膜质地坚韧 ,不易移植。自 1997年至 2 0 0 0年 8月 ,应用鼻侧腱膜固定法矫正内眦赘皮 79例 ,随访 6 5例 6～ 2 4个月 ,均获得了较满意的效果。结论 本方法组织创伤小、术后刀口瘢痕不明显 。

Objective Anatomy of mose was studied on cadavers in order to explore a comparative ideal technique for correction of epicanthus. Methods On the basis of nasal anatomic dissection in 7 adult cadavers, a conception of “lateral nasal aponeurosis” was put forward and a new fixation technique for correcting epicanthus was used cliniclly in the following procedures: incision of the epicanthal fold, cantholysis (widening the canthus) and canthoplasty by pulling the new canthus medially and fixing it on the lateral nasal aponeurosis. Results The lateral nasal aponeursis is solid and immovable. A total of 79 patients were operated by this technique and 65 of them were available for 6 to 24 months′ postoperative follow_up with excellent cosmetic results. Conclusion This technique has lesser tissue damage and more favorable postoperative scar formation. It is a simple and reliable surgical approach for correction of epicanthus.
