早期乳腺癌的X表现

Mammographic Features in Early Breast Cancer

目的 :探讨临床Ⅰ期乳癌的X线征像及诊断。材料和方法 :2 11例乳腺癌中Ⅰ期乳癌中I期乳癌 5 0例 ,Ⅱ～Ⅳ期 16 1例。按美国放射学会 (ACR)创立并推荐的“乳腺影像报告和数据系统” ,对其进行X线征像描述和诊断。结果 :临床Ⅰ期乳癌占乳癌的 2 4%。Ⅰ期乳癌的非肿块影明显多于Ⅱ～Ⅳ期乳癌 ,分别为 2 0 %和 7% ,Ⅰ期乳癌的钙化影少于Ⅱ～Ⅳ期乳癌 ,分别为 30 %和 45 %。Ⅰ期乳癌的X线诊断灵敏度为 74% ,Ⅱ～Ⅳ期乳癌为 93 %。结论 :临床Ⅰ期乳癌的X线表现相对不典型。

Purpose: To study mammographic features in the diagnosis of clinical stage Ⅰ breast cancers. Materials and Methods:The mammograms of 211 patients with breast cancers were reviewed and divided into two groups:50 cases of breast cancer in clinical stage Ⅰ and 161 beyond stage Ⅱ.The X-ray characters of breast cancer in the both groups were described and amalyzed ,according to Breast Imaging and Data System(BIRADS)developed gy American College of Radiology . Results:Clinical stage Ⅰ brdast carcinomawas present in 50(24%)of 211 cases.The frequency of non-mass features in stage I group was significant more than that beyond stage Ⅱ group(20% vs. 7%).Calcificationb was present in 15(30%)of 50 cases in stage I compared with 73(45%)of 161 cases beyond stage Ⅱ.The sensitivity of mammography was lower in stage Ⅰ than beyond stageⅡ breast carcinomas(74% vs.93%).Conclusion :Mammographic fatures in stage I breast cancer are not appeared with typical malignancy .Ultrasound could be a valuable abjunct method for the assessment of early stage breast cancer.
