甲状腺疾病再手术的临床研究

目的 探讨甲状腺疾病再手术的原因和注意事项。方法 对 2 5例甲状腺再手术病例进行研究 ,分析再手术的原因 ,探讨降低再手术发生率 ;总结再手术的方法及术中注意事项 ,探讨减少再手术并发症。结果 2 5例甲状腺再手术病例 ,原发病为甲状腺瘤 15例 ,结节性甲状腺肿 6例 ,甲状腺功能亢进 2例 ,甲状腺癌 2例。首次手术为单侧甲状腺次全切除 8例 ,双侧甲状腺全次切除 8例 ,甲状腺腺瘤切除 7例 ,甲状腺癌根治 2例。再手术原因为首次手术并发症 3例 ,原发疾病复发、转移 2 0例 ,甲状腺新发疾病 2例。再手术治疗全部治愈 ,但发生并发症 5例。结论 甲状腺疾病再手术仍能取得良好疗效 ,但并发症多。必须强调做好首次手术 ,降低再手术率。

Objective To evaluate clinical significance of reoperation for thyroid gland diseases.Methods 25 patients undergoing reoperation for thyroid gland diseases were reviewed retrospectively.Results 25 patients had reoperated for the thyroid gland diseases from 1997 to 2000.The cause of reoperation for the thyroid gland diseases showed that the majority of the patients(80%) was recurrence and metastasis.Although reoperation had higher postoperative complication(20%),but good results can be achieved.Conclusion Selecting correct operative program and taking care of operating skill in first operation may decrease the reoperative mortality and complication.
