侧隐窝小开窗治疗腰椎间盘突出症248例

Treatment of 248 cases of lumbar intervertebral disc herniation with small fenestration operation

目的 探讨手术治疗腰椎间盘突出症的效果。方法 用骨刀行倒U正L形侧隐窝小开窗法共治疗具有单侧下肢症状的腰椎间盘突出症 2 48例。结果 经 1～ 4年随访 ,优 16 0例 ,良 78例 ,可 3例 ,差 7例。结论 手术具有创伤小、恢复早的优点 ,尤其适合于伴有侧隐窝和根管狭窄的腰椎间盘突出症。强调手术不仅要摘除突出的椎间盘髓核 。

HT5”H]Objective [HT5”SS]To review the results of the treatment of lumbar intervertebral disc herniation(LDH) with fenestration operation.[HT5”H]Methods [HT5”SS]248 cases of LDH with symptoms of unilateral lower limb pain were treated with small fenestration operation of lateral recess.The incisional approach to lateral recess was of invert U and upright L shape.[HT5”H]Results [HT5”SS]After 1～4 years follow up,excellent results were obtained in 160 cases,good in 78,fair in 3 and poor in 7.[HT5”H]Conclusion [HT5”SS]The operative method has the merits of small wound and early recovery,which is especially suitable for the treatment of LDH with stenosis of lateral recess and nerve root canal.In order to improve the curative effect,the protruded necleus pulposus of intervertebral disc should be excised and complete decompression of nerve root is also important.
