OK镜引发的重影情况跟踪分析

Clinical analysis of ghost in wearers of OK lenses

目的 :对角膜塑型术 (Orghokeratology,OK镜 )治疗中患者出现的重影现象进行初步分析。方法 :观察随访 9～ 14月的 OK镜治疗者共 5 0例 96眼的重影情况 ,并重点分析重影出现与近视度数、偏位情况、角膜上皮情况等的关系。结果 :本组≤ - 3.0 DS,- 3.2 5～ - 5 .0 DS,>- 5 .0 DS三组患者重影发生率分别为 2 9.6 %、 6 3.8%和 81.8% ,且伴散光者 6 0 %有重影 ,说明随着近视度数的加深 ,重影产生的比率越高 ,且以伴散光者为多 ,重影与角膜上皮擦伤关系不密切 ,但与中央的擦伤有关。重影的产生与次弧下荧光环的形态及镜片偏位有密切关系 ,这关键是我们操作医生未能参与设计或与设计者根本无法沟通 ,不能随个体差异的不同及时进行调整的原因。结论 :OK镜虽可提高远视力 ,但治疗过程中所存在的视觉质量问题 ,值得关注。

The Jinshan Hospital of Shanghai University of Medical Science, Shanghai 200540 Objective: To make a clinical analysis of ghost in the patients who wore orthokeratology lenses (OK lenses). Methods: 96 eyes of 50 cases were observed through 9 to 14 months of follow-up, including the interrelations between ghost phenomena and the diopter, the conditions of the lenses deviations, and the conditions of the scratch on epithelial tissue of cornea. Results: The proportions of ghost in all the 50 cases were 29.6% (diopter≤-3.0DS), 63.8% (-3.25DS = diopter < 5.0DS) and 81.8 % (diopter ( -5.0DS). 60% of the patients with ghost had astigmatism. All this shown that the proportion of ghost would be higher with the rising of diopter, and the compliant would be more common in those patients with astigmatism. The ghost was not closely interrelated to the scratch on epithelial tissue of cornea, but interrelated with the scratch on the middle part of the tissue. The appearance of ghost was closely interrelated to the shape of ring by fluorescent drops under the reverse geometry curve and the lens deviation. Conclusion: There are some problems concerning visual quality arising during the treatment with OK lenses though OK lenses can improve the distance vision.
