儿童先天性髋关节脱位的手术治疗

Surgical therapy for children with congenital dislocation of hip

目的介绍先天性髋关节脱位（ＣＤＨ）手术方法、手术疗效及术后并发症。方法收治ＣＤＨ多种手术病例２５２例，计３７２个髋关节。分３～８岁（Ａ组）及９～１５岁（Ｂ组）两组，进行临床随访分析。结果术后观察２～１３年。随访效果优良率达８８．１７％。结论年龄越小，治疗效果越好，但大龄ＣＤＨ患者不应轻易放弃手术，尤其是发育前儿童。

Objective The operative method for congenital dislocation of the hip (CDH) and its complication as well as clinical effect were introduced. Methods Surgical operations were performed on 252 children (372 hips) with CDH. These children were divided by age into 2 groups for follow up analysis. Results The rate of excellent results reached 88 17% by 2￣13 years follow up. Conclusion The younger the child ,the better the effect. For older children, especially for under developmental children with CDH,operations should not be given up rashly. Open reduction, with femoral rotation and amputation and suitable shorten femur,is a better combination for satisfactory effect.
