全胃肠外营养支持在新生儿腹部手术后的应用

THE APPLICATION OF TOTAL PARENTERAL NUTRITION SUPPORT IN THE NEONATES AFTER ABDOMINAL OPERATION

目的：探讨全胃肠外营养（ Ｔ Ｐ Ｎ）支持对中大型手术后新生儿的影响。方法：自 １９９５～１９９８ 年对 ３０例腹部中大型手术后新生儿采用 Ｔ Ｐ Ｎ 支持疗法，将全营养混合液（ Ｔ Ｎ Ａ）通过周围静脉２４ ｈ 均匀输入，应用时间 ８～２４ ｄ。监测项目包括体重、血常规、尿常规、肝功能、血生化、氮平衡、神经精神发育。结果：痊愈 ２９ 例，死亡 １ 例。应用 Ｔ Ｐ Ａ 支持后 １ 周与应用前相比较，体重、神经精神发育检查评分、血淋巴细胞计数均明显增加（ Ｐ＜ ００１）。结论： Ｔ Ｐ Ｎ 能促进腹部中大型手术后新生儿生长发育特别是神经系统发育，提高手术成功率，减少并发症发生。

Objective:To investigate the influences of total parenteral nutrition(TPN) on the neonates after major operations.Methods:TPN support was applied in 30 neonates from 1995 to 1998.The total nutrient admixture (TNA) was given through peripheral vein in 24 hours.TPN period was 8 to 24 days.Monitering items included body weight,blood routine examination,urine routine examination,liver function test,blood biochemical analysis,nitrigen balance test,neuropsychic development examination.Results:Twenty nine patients recovered and one died.The body weight,nuropsychic development examination score and total lymphocyte count increased significantly ( P <0 01) in 7 days after the application of TPN.Conclusion:TPN can promote the development of body and nervous system in neonates after major operations in abdomen and increases the cure rate of operation and decreases complications.
