外固定在胫骨开放性骨折中的应用

Tbe Use of External Fixator in Open Fractures of the Tibia

本文总结了32例胫骨开放性骨折应用外固定治疗的病例,其中,22例伴有软组织缺损,软组织缺损的面积4cm×4cm～8cm×24cm,4例采用游离植皮修复,18例行皮瓣或肌瓣修复。术后,2例皮瓣远端尖部发生部分坏死,经游离植皮修复,其余病例均取得成功,获得满意的效果。平均随访2.5年,骨折愈合牢固,供区未见明显功能障碍,受区外形较好,肢体功能恢复较满意。

In this paper, 32 patients with open fractures of the tibia treated by external fixator were reviewed. Among them, 22 patients had soft tissue defect. The size of soft tissue defect ranged from 4cm × 4cm to 8cm × 24cm. The free skin graft were used in 4 patients, the skin flaps or myocutaneous flaps were used in 18 patients. Two flaps developed necrosis of distal tip of skin postoperatively, remainder were managed by free skin graft and 16 survived completely with satisfactory results. Average follow-up in the group was 2.5 years. Fractures in the leg healed consistently. Morbidity of the donor sites is minimal. Appearance of recipient sites is satisfactory. The function of the lower extremity is good.
