近端小肠肿瘤出血误行胃大部切除术八例分析

UPPER GASTROINTESTINAL HEMORRHAGE DUE TO TUMOR OF THE SMALL INTESTINE IN 8 PATIENTS WITH GASTRECTOMY

十二指肠和空肠上端肿瘤是引起上消化道出血的少见原因,本文报告17年内8例此类患者,所有病人均以溃疡出血误行胃大部切除术.术后继续出血再次手术,病理证实为肿瘤、延误诊治的主要原因是忽视上消化道出血的少见原因、未充分应用辅助性检查方法、将肿瘤误诊为溃疡以及未及时再探查.

Upper gastrointestinal hemorrhage (UGIH) Caused by tumor of the smallintestine is rare. Eight cases were encountered in our university over a period of seventeenyears. All cases had their privious gastrectomy for UGIH with a preoperative diagnosis ofpeptic ulcer. Recurrent UGIH occurred after the initial surgery and tumors of the proximal small bowel were confirmed by re-operation and pathology,The main reason of misdiagno-sis and erroneous gastrectomy is analzed in this paper.
