抑肽酶对体外循环中红细胞的保护效果

摘要 目的 观察体外循环预充液中加入抑肽酶对红细胞的保护效果。方法 成年杂种犬 12条随机分为对照组和抑肽酶组 ,每组 6条 ,对照组体外循环预充液中仅加入复方氯化钠及 70 6代血浆各 5 0 0ml,抑肽酶组体外循环预充液中除复方氯化钠及 70 6代血浆外 ,另加入抑肽酶 2 .5万U/kg。摸拟临床心内直视手术、心肌缺血再灌注、体外循环过程开胸建立体外循环。抑肽酶组于转机前静脉注射抑肽酶 2 .5万U/kg ,对照组只注射等量的生理盐水。分别于体外循环转机前 ,转机 5、15、45min及开放后 15、30min取静脉血测定血浆丙二醛 (MDA)和游离血红蛋白 (FHB)。结果 抑肽酶组转机 5min后的各次标本MDA、FHB均明显低于对照组。结论 抑肽酶能减少体外循环期间氧自由基的产生 ,对体外循环中红细胞有较好的保护效果。 objective To study the protective effect of Aprotinine on red blood cell during cardiopulmonary bypass(CPB). Methods 12 adult hybrid dogs were divided into aprotinine group ( group A,n=6) and control group( group C,n=6).Aprotinine was administered 25000U/kg intravenously and 25000U/kg was added in priming fluid of CPB mechine in group A. group C was treated as group A exception for not given aprotinine . The levels of malondiadehyde(MDA) and free hemoglobin (FHB) in plasma were observed before CPB, 5min following CPB,15 and 45min after cross clamping and15 、30min after release of aortic cross clamping. Results levels of MDA and FHB of group A were significantly lower than that of group C (P<0.001)after 5min of CPB . Conclusions aprotinine may reduce the production of oxygen free radicals and thus protect the red blood cells during CBP
