刮除植骨治疗骨巨细胞瘤的临床研究

Treatment of Giant-Cell Tumor of Bone with Curettage and Bone-Graft

目的：研究骨巨细胞瘤（ｇｉａｎｔ－ｃｅｌｌ ｔｕｍｏｒ，ＧＣＴ）刮除植骨的局部复发率和并发症。方法：４８例ＧＣＴ选自１９８８～１９９５年我院收治的８２例ＧＣＴ患者，采用囊内刮除植骨，进行临床和放射学随访。结果：经过２８月～１２３月随访，功能优良率８６．９％，局部复发率１６．７％。结论：本研究提示刮除植骨复发率低，可能与肿瘤充分刮除有关。

Objective:The present study was performed to evaluate the rate of local recurrence and complication after treatment with curettage with use of a high-speed burr and reconstruction with autogenous bone graft with or without allograft bone. Method: 48 patients out of 82 GCT cases in our institute from 1988 to 1995 were reviewed to determine the rate of local recurrence aftertr eatment with intralesional curettage with use of high-speed burr and reconstruction with bone graft. There were 27 male patients and 21 female patients. The average age at the time of diagnosis was 30 years. All of the patients were followed clinically and radiographically,and a biopsy was performed if there were any suspicious changes. Results:Through 28 to 123 months of followed up (The mean duration of follow up was 72 months). 86. 9% of the cases achieved excellent or good functional results, 7 patients had a local recurrence. Conclusions:The results of the present study suggest that bone-grafting is better treatment after removal of giant-cell tumors,the low risk of recurrence is related to how thoroughly the tumor is removed.
