男性静止期尿道的超声氧气逆行造影研究

The study of the male urethra in the quiescent period with ultrasound oxygen retrograde urethrography

目的 研究男性正常尿道与异常尿道在静止期的形态走行并探讨其临床意义 ;评价超声氧气逆行尿道造影 (UORU )在男性尿道疾病诊断中的临床应用价值。方法 对 8例正常青年男性 (Ⅰ组 )及 1例尿道癌、2 8例排尿困难的良性前列腺增生患者 (Ⅱ组 )分别行UORU检查。结果 造影前两组研究对象均不能显示静止期尿道的形态走行 ,造影后均能清晰显影。正常男性静止期尿道表现为较光滑、粗细大体一致、气体反射较集中的线状强回声 ,后尿道走行较平直。尿道癌患者表现为低回声肿块内气体反射不集中、见多处弥散分布的气体反射强回声 ,呈“天女散花”征。前列腺增生肥大患者的后尿道均被拉长超过 3cm ,2 7例表现为后尿道曲度增大或迂曲等异常的形态走行。结论 UORU可以完整地显示男性静止期尿道的形态走行 ,有助于分析尿道粘膜的完整性 ,是诊断男性尿道疾病的一项简便、实用的新方法。

Objective To study the form of the normal and abnormal male urethra in quiescent period and its clinical significance and to evaluate the clinical application of ultrasound oxygen retrograde urethrography(UORU) in urethra disease.Methods Eight normal young men(group Ⅰ), 1 patient with urethra cancer and 28 cases with benign prostatic hypertrophy(group Ⅱ) were examined by UORU.Results The urethras in the two groups could not be observed before UORU,but they were all observed after UORU.In groupⅠ,the normal male urethras in the quiescent period were smooth,glossy and bright lines and the tracks of the posterior urethras were flat.In groupⅡ,the gas in the urethra cancer was dispersed,just as the flowers that the fairy lady had scattered here and there.The posterior urethras of the patients with benign prostatic hypertrophy were all lengthened over 3 cm,the tracks in the quiescent period were almost abnormal (27/28),some of the radians of posterior urethra were enlarged or some changed into tortuousness.Conclusions The form of the male urethra in the quiescent period can be completely displayed by UORU,UORU is helpful to analyse the integrality of mucous membrane,it is a new,simple and useful method for the diagnosis of the male urethra disease.
