乳腺髓样癌的超声诊断及其病理基础

Ultrasonographic diagnosis of breast medullary carcinoma and its basis of pathology

目的 探讨乳腺髓样癌的声像图特征及其病理基础。方法 应用彩色多普勒超声检查 11例乳腺髓样癌患者 ( 13个病灶 ) ,观察其声像图表现及血供特点 (包括大小、边界、大体形态、内部回声、后方回声、血流灌注等 ) ,并与病理对照。结果 乳腺髓样癌具有特征性声像图表现 ,包括边界清楚但无包膜、常有分叶、内部回声低而后方回声增强、血供丰富等 ,其上述特征相应的病理基础为 :癌组织主要为大量的肿瘤细胞成分 ,间质成分少 ,肿瘤呈膨胀生长等。结论 超声检查可有助于对本病与乳腺良性肿瘤及其他类型乳腺癌的鉴别诊断。

Objective To investigate the ultrasonographic features and its relative pathologic basis of the breast medullary carcinoma.Methods Breast medullary carcinoma were examined in 11 patients (13 lesions) by color Doppler ultrasound.The relationship between ultrasonographic findings (size,margin,shape,echogenicity and color Doppler blood flow patterns) of tumor and coresponding pathologic basis were studied.Results Ultrasonographic findings of breast medullary carcinoma often showed lobulated,clear margins without capsule and hypoechoic lesions with posterior enhancement color Doppler flow within the tumor.Pathologic findings of breast medullary carcinoma indicated that there were a large number of tumor cells and a few of interstitial cells.The tumor cells grew fast in a short time.Conclusions Ultrasound examination can be belpful in the differentiation of the breast medullary carcinoma from breast bening tumors and the other malignant tumors.
