微创血肿碎吸术治疗老年高血压性脑出血

Evaluation on the therapeutic effects of microinvasive neurosurgery for the hypertensive intracerebral hemorrhage in elderly patients.

目的 评估老年人高血压性脑出血的两种不同治疗方法 ,以寻找最佳治疗方案。方法 采用微创锥颅血肿碎吸术治疗老年人高血压性脑出血 14例 ,以同期传统药物治疗的老年患者 2 8例作为对照 ,所有结果经过统计学分析。结果 经半年以上随访 ,微创锥颅组总有效率为 78.6 % ,(11例 ) ,病死率为 14.3% ,(2例 ) ,药物治疗组分别为 46 .4% ,(13例 ) ,2 5 % (7例 ) ,两组总有效率有显著性差异 (P<0 .0 5 )。结论 微创锥颅血肿碎吸术对老年患者同样安全有效 ,年龄不再是决定手术与否的主要条件 。

Objective [WT5”BZ]To evaluate the effects of two treatments for the elderly patients(aged≥60years)with hypertensive intracerebral hemorrhage(HIH) and makean approach fo optimal protocol of treatment.Methods Fourfeen patients were treated by microinvasive neurosurgery.28 patients as controls were treated by conventional pharmacotherapy.All results were analyzed by statistical test.Results A follow up study of more than six months showed that the general efficacy rate and mortality rate of the surgical treatment group were 78.6% and 14.3%,and those of the control group were 46.4%and 25% respectively;there were significant differences between the two groups ( P <0.05).Conclusion Age should no longer be a major question concerning the indication for surgical treatment,and microinvasive neurosurgery should be one of the most important treatments for the elderly patients with hypertensive intracerebral hemorrhage.The opportunity of aspiration and the prevention of complications are discussed
