外伤性前房积血冲洗手术中粘弹剂的应用

The applying of viscoelastics to traumatic hyphema by surgical treatment.

目的 探讨应用粘弹剂手术治疗外伤性前房积血的效果。方法 对 2 6例Ⅱ～Ⅲ级前房积血部分合并有青光眼角膜血染患者 ,采用标准巩膜隧道式切口 ,术中充分应用粘弹剂保护眼内组织 ,分离粘连 ,游离血块 ,压迫止血 ,冲洗及用显微摄取出前房积血 ,随访 6月。结果 所有病例均一次清除全部前房积血 ,术后视力 0 5以上者占 84 6%。除 4例轻度虹膜后粘连外 ,无其它并发症。结论 应用粘弹剂手术治疗重度外伤性前房积血 ,比不用粘弹剂术后视力恢复好 ,手术并发症大大减少。

Objective To probe the operative treatment effect for viscoelastics to traumatic hyphema.Methods In 26 patients with Ⅱ～Ⅲ traumatic hyphema and some with glaucoma or corneal hemosiderosis,the standard sclero-tunnel incision was adopted.During the operation,hemostasis was done with press,the synechia was seprated,and for protecting the intra-ocular tisssue,the viscoelastics was used plently.And follow up for 6 months.Results All patients the blood was cleaned and the vision increased to 0.5 in 84.6% of the cases after operation.And there was no complication and only low-grade posterior synechia in 4 cases.Conclusion Comparing to routine method,this surgery has many benefits,as better postoperative vision and less complications.
