摘要
目的 :研究 HL A- I、HL A- DR及 ICAM- 1在 JO- RRP细胞膜中表达的免疫及临床意义。方法 :用免疫组化SP法检测 5 4例 JO- RRP,2 0例 AO- RRP和 2 0例喉癌组织中 HL A- I、HL A- DR及 ICAM- 1的表达。结果 :HL A- I在JO- RRP的表达有下降 ,阳性率为 46 .3% ;AO- RRP的 HL A- I表达 (80 .0 % )明显高于 JO- RRP(P<0 .0 1) ,喉癌表达则明显较少 (15 .0 % ) (P <0 .0 2 5 )。 HL A- DR在 6例 (11.1% ) JO- RRP中表达 ,与 AO- RRP及喉癌的表达均无显著性差异 (P >0 .0 5 )。 ICAM- 1在侵袭性 JO- RRP和喉癌中各有 2例阳性表达 ,而非侵袭性 JO- RRP和 AO- RRP中则无一例阳性。结论 :JO- RRP的发病与宿主的免疫密切相关 ,研究 HL A分子的多态性以及 ICAM- 1的表达对了解 JO-RRP的免疫状况有意义。但由于免疫系统的复杂性 ,从单一的免疫指标 HL A- I、HL A- DR、ICAM- 1尚不能对 JO-
Objective: To investigate the immune status and clinical significance of the HLA\|1 (human leucocyte antigen), HLA\|DR and ICAM\|1 (intercellular adhesion molecule) expressions in the tissue of patients with JO\|RRP (juvenile\|onset recurrent respiratory papillomatosis). Methods: The expressions of HLA\|1, HLA\|DR and ICAM\|1 were determined by immunohistochemical S\|P method in the tissues of 54 patients with JO\|RRP (28 males and 26 females, ranging in age from 8 months to 10 years old) (group Ⅰ), 20 patients of adult\|onset RRP (AO\|RRP) (group Ⅱ) and 20 patients with laryngeal carcinoma (group Ⅲ). Results: The expressions rates of HLA\|1 were 64 3% (25/54), 80 0% (16/20) and 15 0% (3/20) in group Ⅰ, group Ⅱ and group Ⅲ, respectively. Result showed significant different on the group Ⅰ and group Ⅱ (P<0.01), group Ⅰ and group Ⅲ (P< 0.025). The expressions of HLA\|DR were 11 1% (6/54), 20 0% (4/20) and 5 0% (1/20) in group Ⅰ,Ⅱ and Ⅲ, respectively, which showed no significant differnce. ICAM\|1 separately expressed on 2 cases of aggressive JO\|RRP and laryngeal cancer. But it didn't show in the non\|aggressive JO\|RRP and AO\|RRP. Conclusion: The role of immune response showed importance in JO\|RRP patients. The study of expression of HLA molecule polymorphism and ICAM\|1 should be beneficial to understanding the host immune response. However, anyone of HLA\|1, HLA\|DR and ICAM\|1 will not definitely estimate the development and prognosis of JO\|RRP.
