左半结肠梗阻一期切除吻合术后并发症的预防

Prevention of complications of primary resection and anastomosis for left colonic obstruction

目的 :总结左半结肠梗阻一期切除吻合术后吻合口漏及腹腔感染的预防经验。方法 :回顾性分析 1992年 10月至2 0 0 0年 10月住院并行结肠一期切除吻合术的低位结肠梗阻 2 8例。结果 :全组术后无吻合口漏和腹腔感染 ,切口感染 2例。平均住院 18d。结论 :合理的手术操作及围手术期处理是预防左半结肠梗阻一期吻合术后发生吻合口漏及腹腔感染的关键。

Objective:To summarize the preventive experience of the anastomotic leakage and intraperitoneal infection following the primary colocolostomy for left colonic obstruction. Methods: Retrospective analyses were made on 28 consecutive patients who suffered from low position obstruction of colon and were hospitalized and underwent the primary colocolostomy between Oct. 1992 and Oct. 2000. Results:There was neither anastomotic leakage nor intraperitoneal infection in all the 28 cases but infection of incisional wound occurred in 2 cases . The average duration of hospitalization was 18 days. Conclusion: Appropriate operative and perioperative management is the key to prevention of anastomotic leakage and intraperitoneal infection following the primany colocolostomy for left colonic obstruction.
