光学相干断层成像与组织形态学检测猴青光眼视网膜神经纤维层厚度的比较 被引量:34

Comparison of nerve fiber layer thickness between optical coherence tomography and histomorphometry in glaucomatous monkey eyes

摘要 目的 应用光学相干断层成像术 (opticalcoherencetomography ,OCT)检测活体青光眼视网膜神经纤维层 (retinalnervefiberlayer,RNFL)厚度 ,并与组织形态学检测结果进行比较。方法 选用Cynomolgus猴 4只眼 ,通过氩激光烧灼小梁网 ,引起眼压升高 ,制作成青光眼动物模型。应用OCT测量视盘周围直径 ,选取直径为 2 4mmRNFL厚度的猴眼进行研究。在完成最后一次OCT测量后 ,将眼球摘除。摘除的眼球置于 10 %甲醛液中固定 ,石蜡包埋做连续切片及HE染色。应用与光学显微镜相联接的计算机图像处理系统 ,测量与OCT扫描相对应点的组织学切片的RNFL厚度。结果 OCT测量RNFL平均厚度为 (72 2± 5 4 5 ) μm ;组织形态学测量RNFL平均厚度为 (70 3± 5 1 1) μm ,未经校正的组织形态学测量平均厚度为 (6 1 1± 44 2 ) μm。两种检测结果差异无显著性(t=1 895 ,P>0 0 5 )。结论 OCT活体测量RNFL的平均厚度与组织形态学测量结果基本相同 ,但OCT活体测量RNFL厚度值与经校正后的组织形态学测量结果更接近。 Objective To correlate the retinal nerve fiber layer (NFL) thickness measured by optical coherence tomography (OCT) in vivo with histomorphometric measurement. Methods Four laser induced glaucomatous eyes of 4 cynomolgus monkeys were studied by OCT scanning a circular area 2 4 mm in diameter around the optic nerve head. The 4 eyes had early or advanced glaucoma. After final OCT measurement, the monkeys were sacrificed and the eyes enucleated. Routine serial section preparations were made, stained with hematoxylin and eosin, and the NFL thickness of 526 sections measured with the aid of a computerized image analysis system was compared with that at the points coincident with the OCT scanned. Results The mean OCT and histomorphometric NFL thickness in glaucoma were (72 2±54 5) μm and (61 1±44 2) μm, after the correction of dehydration 15% (70 3±51 1) μm respectively. Student′s t test showed that the difference was insignificant ( t =1 895, P > 0 05). Conclusions The NFL thickness measured by OCT in vivo correlates well with that by histomorphometry, and the measurement is accurate.
