胆肠Roux-Y吻合术后再手术原因及处理

CAUSES AND TREATMENT OF REOPERATION AFTER ROUX-Y CHOLANGIOJEJUNOSTOMY

目的 探讨胆肠Roux -Y吻合术后再手术的原因。方法 对 12例胆肠Roux -Y吻合术后再次手术的患者进行回顾性分析。结果 肝内胆管狭窄合并肝内残余结石是再次手术的主要原因 ,胆肠吻合口相对狭窄、以及空肠袢较短、空肠 空肠吻合口形态不佳也是再次手术的重要因素。结论 行胆肠Roux -Y吻合术必须严格掌握手术适应证 ,并有一定的抗返流措施。

Objective To study the causes of the reoperation after Roux-Y cholangiojejunostomy.Methods A retrospective analysis was made om the clinical data of 12 cases of reoperation after Roux-Y cholangiojejunostomy.Results The mostly cause of reoperation after Roux-Y cholangiojejunostomy was stricture of intrahepatic bile duct accompanied with remains stones.The other causes were relatively cholangiojejunostomy anastomatic stenosis,inadequate length of jejunum limb and unreasonable jejunojejunostomy anastomosis.Conclusion It is necessary to master rigidly the indication of Roux-Y cholangiojejunostomy,which must have antirefluxing result.
