能量多普勒显像在睾丸肿瘤诊断中的作用

The Role of Power Doppler Imaging in the Diagnosis of Testicular Tumor

目的 探讨能量多普勒显像 (PDI)在睾丸肿瘤诊断中的作用。方法 连续积累睾丸肿瘤病人 2 5例。正常对照 30例 ,异常对照睾丸炎 15例。二维超声观察睾丸肿瘤的形态学改变 ,PDI显示睾丸肿瘤有无血流、血流的多少、形态与分布 ,脉冲波多普勒测量异常血流最大速度 (PSV)和阻力指数 (RI)。结果 PDI显示 3例睾丸肿瘤为少血供 ,其余均为多血供 ,并呈条带状、树枝状及网状分布 ,且 81.8% (18/ 2 2例 )的睾丸肿瘤血流形态不规则。睾丸肿瘤的RI和PSV与正常睾丸相差显著 (P <0 .0 1) ,与睾丸炎的RI明显不同 (P <0 .0 1) ,而PSV无明显差异 (P >0 .0 5 )。结论 PDI显示睾丸内局灶性多血管改变且血管形态不规则 ,有助于睾丸肿瘤的诊断和鉴别诊断。

Purpose To evaluate the role of power Doppler imaging in the diagnosis of testicular tumor. Methods 25 cases with testicular tumors, 30 healthy men and 15 patients with testicular inflammation were studied. Two dimensional ultrasonography was used to show the morphological changes of testicular tumor, its blood flow was showed by power Doppler imaging and was measured by spectral Doppler. Results The PDI showed that 3 tumors were hypovasculars and others were hypervasculars, and that there was irregular blood flow patterns in 18/22 cases. The RI of testicular tumor was significantly different from that of controls and orchitis; It's PSV was significantly different from controls's. Conclusion The regional hypervasculars in the testis showed by PDI is helpful the in diagnosis of testicular tumor. [
