介入性超声在经宫颈子宫肌瘤切除术中的应用

Application of Contrast Ultrasound in Transcervical Resection of Myoma

目的 :评价介入性超声在经宫颈子宫肌瘤切除术中的应用价值。材料和方法 :30 4例子宫粘膜下肌瘤和子宫壁间肌瘤的患者在超声双向对比法监视下用宫腔电切镜行经宫颈子宫肌瘤切除术。结果 :切除子宫粘膜下肌瘤 2 2 5例 ,完全及部分切除子宫壁间肌瘤 79例。其中一次手术 30 1例 ,二次手术 3例 ,术后使用缩宫素 41例 ,发生水中毒 2例。结论 :介入性超声可有效的监导手术过程、观察膨宫效果及提示术中使用缩宫素 ,是监导经宫颈子宫肌瘤切除术的安全、有效方法。

Purpose: To evaluate application value of contrast urtrasonic study in transcrvical resection of myoma. Materials and Methods: 304 patients with submucous myoma and leiomyoma underwent transcervical resection of myoma using hysteroresectoscope under dual contrasts ultrasonography. Results: Submucous myoma in 225 patients and leiomyoma in 79 patients were resected or ablated under ultrasonographic monitoring. 3 patients had secondary operation, 41 patients took oxytoxin during the operation, and 2 cases had water intoxication. Conclusion: Contrast ultrasonic study was an efficient and safe method for monitoring transcervical resection of myoma, and of value in guiding operation, observing the distending pressure and prompting use of oxytoxin during operation.
