胃肠道粘膜相关淋巴组织淋巴瘤临床分析

A Clinical Analysis of Gastrointestinal Lymphoma of Mucosa Associate Lymphoid Tissue

目的 :通过分析胃肠道粘膜相关淋巴组织淋巴瘤的临床特征 ,以利于早期正确诊断、治疗、改善愈后。方法 :复习 1 995年 4月至 1 998年 1月 ,1 8例胃肠道原发恶性淋巴瘤进行分析。结果 :1 8例胃肠淋巴瘤 ,均经手术或病理证实 ,均为NHL。其中 1 7例为B细胞系 ,1例为T细胞系 ,胃 1 2例 ,小肠 (回肠 ) 2例 ,结肠 3例。按AnnArbor分期 ,Ⅰ期 1 2例 ,Ⅱ期 6例。治疗以手术加COP CHOP方案为主 ,其中 ,3例以化疗加放疗为主。全组 3年无瘤生存率 88.8% ( 1 6 1 8)。结论 :胃肠道恶性淋巴瘤好发于胃 ,其次为回肠 ,结肠少见 ,以NHL、B细胞系为主 ,确诊依赖于病理 ,以综合治疗为主 ,愈后较好。

Objective:To study clinical characteristics of gastro intestinal lymphoma for correct diagnosis and treatment in early stage,and improving prognosis. Method:Between1995.4 and 1998.1,18 cases of gastro intestinal lymphoma were treated in our department. Results:18 cases with gastro intestinal were pathologically diagnosed.All cases belonged to non Hodgkin's lymphoma.Among them,17 cases were of B cell and 1 case of T cell. 12 cases were of Ⅰstage and 6 cases of Ⅱstage.15 cases were treated with postoperational chemotherapy,while 3 cases,with chemotherapy plus radiotherapy.3 year disease free survival rate (DFS) was 88.8%(16/18). Conclusion: The diagnosis of gastrointestinal lymphoma depends upon pathological examination .Postoperational chemotherapy plus radiotherapy are important.The prognosis is good.
