摘要
目的 :观察富马酸比索洛尔对扩张型心肌病致心衰的疗效。方法 :6 8例病人常规治疗 2周后加服小剂量富马酸比索洛尔 4周 ,比较治疗前后心功能参数、2 4h室性心律失常总数及临床效果。结果 :治疗后心率明显低于治疗前 (P <0 .0 5 ) ,心脏彩超心功能参数显著改善 (P <0 .0 5 ) ,治疗后 2 4h室性心律失常总数显著下降 (P <0 .0 1) ,临床总有效率 92 .6 5 %。结论 :小剂量富马酸比索洛尔能改善扩张型心肌病病人心功能 ,减少室性心律失常发生 ,改善心衰症状 ,小剂量慎用值得临床选择。
Objective:To observe the effect of bisopr olol fumarate in the treatment of heart failure due to dilated cardiomyopathy.Methods:Conventional therapy was used for two weeks ,then small dose of bisoprolol fumarate was added for an other four weeks.The therapeutic effects,heart rates,blood pressure,ejection fra ction,EF slope,24 hour total number of ventricular ectopic beats(VEB) and clini cal results were observed and compared in 68 patients.Results:The heart rate was 92.7±8.6 bpm vs75.4±5.9 bpm(P<0.05)before and after treatment,Blo od pressure was 137.25±16.25/ 84.35 ±9.75 mmHg vs129.75±15.55/84.25 ±9.5 5 mmHg(P>0.05),ejection fraction was 0.30±0.07 vs0.40±0.09(P<0. 05),EF-slope was 34.28±10.11 vs56.35±9.05(P<0.05).The numbers of V EB were 1 097±526 vs225±119(P<0.01).The clinical total effective rate was 92.65%. Conclusion:Small dose of bisoprolol fumarate can imp rove systo diastolic function of dilated cardiomyopathy,reduce the VEB frequenc y and improve the symptoms of heart failure.
