结肠癌并急性梗阻的手术治疗方法改进探讨

Improvement on surgical treatment of conlon cancer with acute obstruction

目的 :探讨结肠癌并急性肠梗阻的术式选择。方法 :急诊手术治疗右半结肠癌并急性肠梗阻 11例 ,行一期切除、端侧吻合 ;左半结肠癌并急性肠梗阻 2例 ,术中施行减压灌洗 ,一期切除肿瘤吻合 ,肛门放置肛管。结果 :手术均获成功 ,术后效果良好。术后并发症 :1例切口感染。结论 :右半结肠癌并急性肠梗阻行一期切除、端侧吻合。左半结肠癌并急性肠梗阻如全身状况较好 ,肠壁血运良好 ,采用术中减压灌洗 。

OBJECTIVE:To explore the selection of operation on acute obstruction caused by colon cancer. METHODS: In emergency oberation ,one stage of the right hernicolectomy was performed in 11 patients, the top and side under went anastomosis; one stage resection of the left hemicolon was performed in 2 patients, undergoing intraoperative decompressed irrigation, the anal catheter was placed. RESULTS: All operation were successful, post operative effect were better. There was one postoperative complication for incision infection. CONCLUSION:One stage resection was performed for the right hemicolon tumor with acute obstruction, top and side underwent arastomosis one stage resection combined with intraoperative decompressed irrigation was performed for the left hemicolon tumor with acute obstruction, if the patient was in good case, and blood circulation of the wall of colon was normal.
