老年人慢性充血性心力衰竭与心率变异的关系

RELATIONSHIP OF CHRONIC CONGESTIVE HEART FAILURE AND HEART RATE VARIABILITY IN THE ELDERLY

目的 :探讨老年患者慢性充血性心衰 (CHF)与心率变异性 (HRV)的关系。方法 :用 2 4h动态心电图监测分析 5 0例慢性充血性心衰老年患者及 5 0例无心衰老年人的HRV时域指标变化。结果 :心衰组其HRV时域指标明显低于无心衰组 (P <0 .0 1) ,且R -R间期标准差随着心衰程度的加重和年龄的增加而降低 (P <0 .0 5 )。结论 :老年慢性充血性心衰患者的HRV时域指标明显降低 ,且与心衰的程度。

Objective:To investigate the relationship of chronic congestive heart failure (CHF) and heart rate varibility (HRV) in the elderly.Methods:The study group comprised 50 patients with chronic congestive heart filure(68.7±9.8 years old),the control group involved 50 patients without CHF(66.5±9.2 years old).Results:The HRV was significantly lower in CHF (NYHA function class ⅡtoⅣ)group than in control group (P<0.01)and the standard deviation of all normal R-R intervals(SDNN) decreased significantly with the aggrevation of CHF and the increase of age.Conclusions:The time-domain HRV indices is significantly decreased in chroinc CHF patient and show negative correlated to degree of CHF and age.CHF is associated with autonomic dysfunction,HRV is a better predictor of CHF than other conventional clinical measurements.Autonomic dysfunction may play an important role in the pathophysiology of CHF.
