摘要 目的 探讨骨巨细胞瘤中多核巨细胞和单核基质细胞的起源及在肿瘤中的作用。 方法 应用免疫组织化学方法检测 5 0例骨巨细胞瘤手术标本的增殖细胞核抗原 (PCNA )、巨噬细胞抗原 (CD6 8)、波形蛋白、抗糜蛋白酶 (AACT)、抗胰蛋白酶 (ACT)以及溶菌酶的表达和分布状况。 结果 多核巨细胞主要表达 CD6 8、AACT、ACT以及溶菌酶 ,无一表达 PCNA;单核基质细胞可分为两种类型 ,以纤维母细胞型占优势 ,主要表达波形蛋白和PCNA;组织细胞型主要表达 CD6 8、AACT、ACT以及溶菌酶。 结论 多核巨细胞并非肿瘤细胞 ,它可能来自单核巨噬细胞系统。单核基质细胞中的纤维母细胞型为肿瘤的主要增殖成分 ,可能来自间充质。 Objective\ To determine the origin and function of multinucleated giant cells and mononulear stroma cells in giant cell tumor of bone.\ Methods\ 50 cases of giant cell tumor of bone were evaluted for their proliferative cell nuclear antigen(PCNA), CD68, vimentin, antitrypsin(ACT), antichymotrypsin(AACT) and lysozyme by using immunohistochemical SP method.\ Results\ Multinucleated giant cells were mainly labeled by CD68, ACT,AACT and lysozyme, but were unlabeled by PCNA. \ Mononuclear stroma cells were either fibroblast\|like cells or histocyte\|like cells.\ The fibroblast\|like cells showed an intense immunoreactivity for vimentin and PCNA, whereas the histocyte\|like were intensely reactive for CD68, lysozyme, ACT and AACT.\ Conclusion\ The fibroblast\|like cells were the major proliferating cells in the giant cell tumors which may originate from the mesenchymal fibroblasts.\ The multinucleated giant cells do not come from a proliferating population of cells but may be a fusion product of histocyte\|like cells which possibly derived from the mononuclear macrophage system.\;
