实验性兔心肌缺血心肌波纹状变与缺血预适应的初步研究 被引量:1

摘要 目的 探讨兔急性心肌缺血时心肌细胞波纹状变的病理学意义及其与缺血预适应的关系。方法 对健康家兔经静脉注入麦角新碱致其冠状动脉痉挛心肌缺血 1小时和缺血后再灌注72小时 ,分别观察心肌缺血 1小时和缺血再灌注的心肌组织病理学变化。结果 缺血 1小时的兔心肌组织出现典型的有节律性的心肌纤维波纹状变和收缩带形成 ,其超微结构变化主要为心肌线粒体肿胀、嵴断裂 ,肌节凝聚和松驰 ,Ⅰ带缩窄和增宽 ,但心肌膜结构完整。缺血再灌注 72小时的心肌组织变化不明显 ,近似正常心肌。结论 心肌波纹状变和收缩带形成是急性心肌缺血的早期形态学基础 ,实验证明该变化是可逆的 ,提示该变化可能是心肌组织对缺血缺氧的一种功能性反应 ,并可能与缺血预适应有关。 Objective In order to study the histopathological significance of myocardial wava front phenomenon and its relation with ischemia preconditioning in period of myocardial ischemia of rabbits.Methods The coronary artery was spasmed with intravenous perfusing ergotocin for 1 hour and reperfusing 72 hours in rabbits.The morphologic changes of myocardial cell were investigated with light microscope and electron microscope.Results The regular wave front phenomena and contracted bands of myocardial fibver were found under light microscope.The primary changes of ultrastructures are as follows:accumulation, swelling and crista breaking of mitochondria,myocomma coagulation and relaxation,and constricted or widened Band I,but the myolemma was intact,and necrosis injury of myocardial cell was found in ischemia groups(1 hour).There were not evident characteristic lesions in reperfusion(72 hours) and control groups.Conclusion The results suggest that the wavefront phenomena and myocardial contracted bands are the histopathologic evidence of early ischemia,but these changes are reversible.We believe that these changes may be a functional reaction to ischemia and hypoxia,and it may be related to ischemia preconditioning of myolemma.
