原发性腹膜后肿瘤手术治疗的体会(附77例报告)

Experience in Surgical Treatment of Primary Retroperitoneal Tumors in 77 Cases

目的：分析总结原发性腹膜后肿瘤的手术治疗情况。方法：分析７７例经手术治疗的原发性腹膜后肿瘤的临床资料，包括手术切除率、手术范围、再次手术情况和组织病理学。结果：良性肿瘤３４例，恶性肿瘤４３例；完全切除率。部分切除率和联合脏器的切除率分别为７４．０％、１０．４％和２３．４％；其中，良性肿瘤的完全切除率为８５．３％，恶性肿瘤为６５．１％，两者有显著差异。复发肿瘤的完全切除率达６３．６％。结论：肿瘤浸润至大血管和多脏器是影响切除率的主要因素，而复发肿瘤仍有可能完全切除。

Objective: To analyse the clinical data of cases of primary retroperitoneal tumor(PRT) admitted during the period from February 1980 to November 2000. Methods: The clinical data of 77 cases of PRT, including the resection rate, the extent of surgery, the rate of reoperation and the histopathologic findings were collected and analysed. Results: Thirty-four cases were benign, and 43 malignant. The rates of complete resection, partial resection and adjacent organ(s) resection were 74.0%, 10.4% and 23.4% respectively. The rates of complete resection in benign and malignant PRT were 85.6% and 65.1%, with statistical difference. Complete resection was successfully carried out in 80% of recurrent cases. Conclusions: The leading factors which prevent complete resection are large vessel and/or multiple organ(s) involvement; however, complete resection could still be expected in many recurrent cases.
