大鼠门静脉右支结扎和栓塞的对比研究 被引量:8

摘要 目的 观察门静脉右支结扎引发的近端完全阻塞和碘油酒精混合物引发的远端栓塞之后的肝脏形态变化和功能改变。方法  2 5只大白鼠随机均分成 5组 ,对 1、2组和 3、4组大白鼠的门静脉右支分别采用碘油酒精2∶1混合物进行栓塞和单纯结扎处理。第 5组为正常对照组。术后第 14d和 2 1d第 1、3组和第 2、4组动物分别被处死 ,剖腹观察肝脏各叶的形态变化 ,并进行肝叶分离和称重 ,计算右叶与全肝的重量比 (比例 1)和肝脏与体重的重量比 (比例 2 )。然后用t检验方法将各组结果与对照组进行统计学比较分析。结果 术后两组大白鼠肝右叶均见明显变小 ,但第 1、2组 (栓塞 )肝脏右叶形态较不规则 ,表面见大小不等淡黄色小瘢痕 ,第 3、4组 (结扎 )的肝脏右叶明显缩小 ,但表面光滑整齐 ,形态呈均匀变小。统计学分析显示两组之间无显著差别。结论 门静脉右支的近端结扎和末梢栓塞虽然导致肝叶萎缩的原理和形态学变化有所不同 ,但引发的萎缩在程度上无显著差别 ,在右叶萎缩的同时 ,其他肝叶发生代偿性增生 。 Objective To evaluate the morphological and functional changes after proximal complete block by ligation or distal embolization by Lipiodol-ethonal 2∶1 mixture at the right portal vein branch.Methods Twenty-five SD rats were randomly divided into 5 groups,5 rats each. Right portal veins were distally embolized by Lipiodol-ethonal 2∶1 mixture in group 1?2 rats and were ligated in group 3 and 4. Five rats served as a control group. Rats of group 1?3 and rats of group 2?4 were sacrificed to inspect the morphological changes of each lobe at 14 and 21 s days after the procedure. Each lobe of the liver was dissected and weighted to obtain the weight ratios of the right lobe to the whole liver and the whole liver to the body. The results of four groups were analyzed and compared to each other and to those of the control group using statistical Student 't' test.Results At 14 or 21 days after the procedure, the right lobes of the rats in group 1 and 2 were atrophied apparently with lots of spotty yellow scars in different size. In the rats from group 3 and 4, the right lobes were significantly atrophied but the morphology had no significant change. By statistical analysis, no significant difference was found between the embolization group and the ligation group ( P > 0.05 ). Conclusion Right portal vein branch embolization using Lipiodol-ethanol 2∶1 mixture and proximal ligation can induce similar extent of atrophy of the lobe although the morphology was different. The rest lobes of the liver become hypertrophied and the weight of the whole liver remains the same although the right lobe is atrophied.
