脑梗塞患者PT、APTT、FIB检测的临床意义

Studies on the clinical significance of measurement for PT,APTT and FIB in the cerebral infarction patients

目的 :探讨脑梗塞 (CI)患者血液凝血状态的改变和凝血指标检测的临床意义。方法 :使用美国欧家农四通道血凝仪检测 118例脑梗塞患者和 30例健康者的凝血指标 :血浆凝血酶原时间(PT) ,活化部分凝血活酶时间 (APTT)和纤维蛋白原 (FIB) ,并计算其国际标准化比率 (INR) ,其中 35例患者服药前、后还分别进行了凝血指标的对比检测。结果 :脑梗塞患者的PT、INR两项指标与正常对照组比较 ,有非常显著性差异 (P <0 .0 1) ;而APTT、FIB两指标相比较 ,则差异不显著 (P >0 .0 5)。同时 ,患者服药前、后的PT、INR相比较 ,有非常显著性差异 (P <0 .0 1) ,而APTT、FIB相比较 ,则差异无显著性 (P >0 .0 5)。结论 :脑梗塞患者血液的凝血状态有明显改变 ,可能为凝血因子水平、活性及功能改变引起 ,该结果为脑梗塞的预防、治疗及发病机制的研究提供了依据。

Objective: To analyse the variation of the coagulaton state and its clinical significancein the cerebral thrombosis. Methods: 118 venous blood samples from the patients were measured for plasm prothrombin time(PT), activated partial thromboplastic time(APTT), fibrinogen(FIB) and were calculated INR by automated coagulation analyzer, 35 of them were measured for PT, APTT and FIB with and without taking medicine respectively. At the same time, 30 venous blood samples from the normal subjects were measured as control. Results: The PT and INR values in the cerebral thrombosis showed clinically significant differences compared with the healthy control's (P<0.01). The PT and INR values also showed clinically significant differences between the patients with and without taking medicine(P<0.01). However, the differences in APTT and FIB values were not significant (P>0.05). Conclusions: The coagulation state in the cerebral thrombosis varied significantly. It may be due to changes of the level and /or activity and /or function of some coagulaiton factors. These results provide some guidance for prevention and treatment of cerebral thrombosis, and further studies should be followed.
