儿童间变型大细胞非霍奇金淋巴瘤临床特点及疗效分析

Clinical features and therapeutic evaluation in children with anaplastic large cell non-Hodgkin's lymphoma

为探讨儿童间变型大细胞非霍奇金淋巴瘤(CALCL)临床特点、方案选择及疗效分析,对4例儿童间变型大细胞性非霍奇金淋巴瘤患儿进行临床诊断、免疫分型、治疗、随访及疗效总结。结果:4例CALCL患儿中,CD30^+3例,T细胞型3例,B细胞型1例;部分缓解1例。完全缓解3例,其中1例CR 7月后脑膜复发。结论:间变型大细胞淋巴瘤(ALCL)为非霍奇金淋巴瘤的一种特殊类型,临床表现多样,与其他恶性淋巴瘤相比恶性程度稍低,较少累及骨髓,预后较好,但与免疫分型及临床分期密切相关。

To explore clinical features and therapeutic evaluation of childhood anaplastic large cell non-Hodgkin's lym-phoma (CALCL), all clinical data were summarized and analyzed in 4 children with CALCL. The results of immunopheno-types showed that there were three cases with CD30+ , one case with CD30- , three cases with T lymphocyte type and one with B-lymphocyte type. Except for one case with partial remission, three cases with CALCL got complete remission (CR) but one of three cases had a relapse in meninges after 7-month CR. It is concluded that CALCL is a Kind of special lymphoma with various clinical features. It is rare to have the involvement of bone marrow comparing to other types of lymphoma. There is usually a good prognosis although CALCL is closely correlated with different immunophenotypes and clinical stages.
