摘要
目的 :探讨PCR技术对中枢神经系统白血病的早期诊断。方法 :采用细胞分子生物学基因诊断技术———聚合酶链反应技术 (PCR)检测脑脊液中白血病淋巴细胞的IgH和TCRγ 基因重排。结果 :确诊组 15例脑脊液标本用PCR均检测到基因重排 ,检出率为 10 0 % ,未确诊组 10例脑脊液标本中有 4例PCR检测出重排基因 ,该 4例患者临床上均尚未出现脑膜白血病的症状和体征。结论 :PCR检测脑脊液中白血病淋巴细胞基因重排有利于CNLL的早期诊断 。
Objective:To test whether the central nervous system leukemia could be detected out earlier by polymerase chain reaction(PCR).Methods:The immunoglobulin heavy chain(IgH)and T cell receptor gamma(TCR γ) gene rearrangements in central nervous system leukemia cells were detected out by polymerase chain reaction(PCR)and DNA gel electrophoresis.Results:The B cell CDRⅢ IgH and T cell TCR γ gene rearangements revealed in all 15 CSF of diagnosed group,the positive rate 100%,and also revealed in 4 of 10 CSF of diagnosis-unconfirmed group.Conclusion:PCR can more effectively and much earlier establish the diagnosis of gene rearrangement of the central nervous system leukemia cells,particularly can be useful for patients with minimal disease in the CSF.
