腰椎间盘移位症术式的选择与疗效分析

Comparison and choice of three different surgical treatments of lumbar intervertebral disc displacement

目的与方法 对 76 5例腰椎间盘移位症的 3种术式 (椎板间开窗、半椎板切除、全椎板切除髓核摘除 )疗效进行总结分析。结果 3组患者术前在年龄结构 ,病变范围 ,症状方面的差异无显著性意义 ,而采取不同的术式进行治疗后 ,3组患者两两之间在症状和体征方面的差异有显著性意义。结论 3术式中椎板间开窗组的疗效最好 ,并发症最少 ;而全椎板切除组的疗效最差 ,腰椎不稳 ,肌萎缩和硬脊膜压迫 ,粘连等并发症的发生率最高。半椎板切除组的情况介于中间。

Objective To investigate the effect of three different surgical treatments of lumbar intervertebral disc displacement.Methods A total of 765 patients were treated by fenestration,semilaminectomy,and total-laminectomy ,respectively.Results There was no significant difference between the 3 groups in symptoms and signs before operation,but after treatment, significant difference was seen between the 3 groups.Conclusion The fenestration lumbar discectomy has the best outcome and the least complications,whereas the total-laminectomy discectomy bears the worst outcome and the frequently encountered complcations such as instability of lumbar vertebrae,dura compression and muscular weakness.
