雄激素受体基因微卫星CAG多态性与前列腺癌的分期分级

CAG microsatellite polymorphisms of androgen receptor gene and the stage and grade of prostate cancer

目的 研究雄激素受体 (AR)基因三核苷酸CAG微卫星多态性与前列腺癌 (PC)分期和分级的关系 ,探讨其与肿瘤生物学特性的相关性。方法 运用聚合酶链反应 单链构象多态性分析法(PCR SSCP) ,对 37例PC标本的AR基因CAG微卫星数量进行测定 ,并进行不同病变期别、分化级别间的比较及统计学分析。结果 AR基因CAG微卫星数量在病变B期与C～D期分别为 2 5 .0 4± 1.88和 2 2 .14± 2 .6 4(P =0 .0 0 2 ) ;其短的AR基因CAG(数量 <2 2 )的分布为 :B期 13.0 % (3/2 3) ,C～D期2 8.6 % (4/14) ;其长的AR基因CAG(数量≥ 2 2 )的分布为 :B期 87.0 % (2 0 /2 3) ,C～D期 71.4% (10 /14) ,差异有显著性 (P =0 .0 2 9)。AR基因CAG微卫星数量随PC病变分化程度的增高而增加 ,低分化为 2 3.6 4± 2 .5 6 ,中分化为 2 4.2 9± 3.45 ,高分化为 2 5 .6± 1.6 7,在高分化与低分化间差异有显著性 (P=0 .0 47)。短组CAG微卫星的分布比例随病变分化程度的升高而降低 ,长组CAG微卫星的分布比例随病变分化程度的升高而增加。结论 AR基因CAG微卫星数量的多少可能是构成前列腺癌不同生物学特性的因素之一 ,短的CAG微卫星的AR基因可能与恶性度较高的前列腺癌表型有关。

Objective To explore the relationship between the CAG mirosatellite in androgen receptor(AR)gene and the stage and grade of prostate cancer(PC).Methods The number of CAG repeats in exon 1 of the AR gene was measured in 37 PC tissues by PCR SSCP analysis, and its association with stages and grades of PC determined.Results The average number of CAG repeats of AR gene differed significantly between PC in stage B (25.04±1.88) and in stage C～D (24.14±2.64)( P =0.002). The frequency of short CAG(<22) was 13.0% (3/23) in stage B and 28.6%(4/14) in stage C～D, while that of long CAG(≥22) was 87.0%(20/23) and 71.4%(10/14) ( P =0.029 ), respectively. The average number of CAG repeats increased with the decrease in tumor cell differentiation. The frequency of short CAG repeats dropped with the increase in tumor cell differentiation while the opposite was true for that of long CAG repeats.Conclusion The number of CAG repeats in AR gene may be one of the factors determining the biologic characteratics PC. Short CAG repeats may signify aggressiveness of PC.
