硝苯地平控释片改善稳定性心绞痛患者一氧化氮对淋巴细胞[Ca^(2+)]i的调节作用

Nifedipine improves the effect of nitric oxide on [Ca^(2+)]i of plasma lymphocyte in patients with stable angina pectoris

目的 观察硝苯地平控释片 (商品名拜新同 )对稳定性心绞痛患者的疗效及对外周淋巴细胞 [Ca2 + ]i的影响。方法 对 6 1例稳定性心绞痛患者进行为期 8周的随机单盲平行对照试验。硝苯地平控释片组系在对照组常规治疗的基础上加服硝苯地平控释片 30mg/d。治疗前后测两组患者外周淋巴细胞 [Ca2 + ]i和一氧化氮 (NO)。结果 (1)两组心绞痛发作次数及硝酸甘油耗量均明显下降 (P <0 0 1) ,硝苯地平控释片组优于对照组 (P <0 0 5 ) ;(2 )经过 8周治疗后硝苯地平控释片组较对照组外周血淋巴细胞内源性NO明显增加 (P <0 0 1) ;同时也较对照组淋巴细胞 [Ca2 + ]i降低 (P <0 0 1) ;(3)治疗后淋巴细胞 [Ca2 + ]i随着NO升高而下降 ,二者呈显著负相关 (r =- 0 6 2 83,P <0 0 0 1)。结论硝苯地平控释片明显改善稳定性心绞痛患者症状 ,逆转细胞内钙超负荷。

Objective To evaluate the effects of Nifedipine GITs on patients with stable angina pectoris and its effects on [Ca 2+ ]i of plasma lymphocyte. Methods The study was designed as a randomized, single blind, parallel controlled trial. Sixty one patients with stable angina pectoris were divided into two groups (Thirty one patients in control group and thirty patients in Nifedipine GITS group) receiving routine therapy (excluding calcium antagonists) or routine therapy plus Nifedipine GITS (30mg per day, once a day) for 8 weeks. Nitric oxide (NO) and [Ca 2+ ]i of plasma lymphocyte were measured before and after the treatment.Results (1) The attacks of angina pectoris and the consumption of nitroglycerin reduced significantly in both groups ( P <0.01), which was more remarkable in Nifedipine GITS group than in control group ( P <0.05); (2) NO level of lymphocyte was significantly increased ( P <0.01) in Nifedipine GITS group, which was higher than that of control group ( P <0.01); (3) Free calcium ([Ca 2+ ]i) level of lymphocyte was decreased in Nifedipine GITS group ( P <0.01), which was lower than that of control group ( P <0.01); (4) There was a significant negative correlation between NO level and [Ca 2+ ]i level ( r =-0.628 3, P <0.001)after eight weeks treatment. Conclusion Nifedipine GITS was able to improve clinic symptoms in patients with stable angina pectoris and inhibited the over loaded [Ca 2+ ]i of plasma lymphocyte.
