直肠癌围手术期抗生素预防性应用205例报告

Preventive application of antibiotics in perioperative period of rectal cancer:report of 205 cases.

目的 探讨围手术期预防直肠癌术后感染的用药方案。方法 对 2 0 5例直肠癌病人围手术期的抗生素应用进行回顾性研究。病例分为三组 ,A组为 1991年 5月至 1993年 2月典型的全程应用抗生素病例 ,B组为1996年 3月至 1997年 2月短期应用抗生素病例 ,C组为 2 0 0 0年 3～ 9月短期肠道准备病例。结果 三组病例术后第 3日体温、白细胞总数、切口愈合情况、感染率和肠液培养、血培养阳性率等差别均无显著性 ,C组病人术前服用抗生素用量明显减少 ,B组、C组病人抗生素用量较A组明显减少。结论 直肠癌围手术期短期预防性应用抗生素安全、经济、可靠 ,值得临床认真执行。

Objective To discuss the regimen of perioperative prophylactic administration of antibiotics to the rectal cancer patients.Methods The use of antibiotics in 205 rectal cancer patients in perioperation period was retrospectively studied.Group A patients were cases of long term antibiotics usage during May 1991 to February 1993 while group B patients were cases of short term antibiotics usage during March 1996 to February 1997,and group C patients were cases of using one-day bowel preparation method before operation during March 2000 to September 2000.Results No differences were seen in temperature,white blood count,wound healing,infectious rates,bowel fluid and the positive rates of blood culture between the three groups.Oralantibiotics taking were decreased in group C.The dosage of antibiotics used were decreased in group B and group C(P<0.01).Conclusion Perioperatire short term usage of antibiotics is safe,effective and economical,and the regimen should be carried out seriously in clinical treatment.
