超声微型探头在大肠癌术前分期中的应用

Preoperative staging of colorectal carcinoma with ultrasound mini-probe.

目的 评价超声微型探头 (mini- probe-sonography ,MPS)对大肠癌术前分期的应用价值。 方法 对5 0例大肠癌病人进行前瞻性的术前肠镜下MPS检查 ,全部病人进行手术治疗 ,将术前分期与术后病理结果进行对比分析。结果 MPS对肿瘤浸润程度 (T)的符合率T1、T2、T3、T4分别为 75 %、80 %、88%、6 7% ,总符合率为84% (P <0 0 1)。区域淋巴结受累程度 (N)的敏感性为 79% ,特异性为 91% ,阳性预测值为 92 % ,阴性预测值为77% ,总符合率为 84% (P <0 0 1)。结论 MPS对大肠癌提供较为准确的术前分期 ,提示肿瘤浸润程度及淋巴结转移 ,对制定治疗方案有指导意义。

Objective To evaluate the accuracy of mini-probe sonography(MPS)in preoperative staging of colorectal carcinoma.Methods 50 patients with colorectal carcinoma were examined preoperatively with MPS(12MHz 244mm),comparison was made between MPS findings and pathological results after surgical resection.Results The accuracy of MPS for depth of invasion(T category)was 75%,80%,88%,67% in T1,T2,T3,T4 respectively,and 84% in all cases(P<0.01).The accuracy of MPS for nodal staging(N category)was 84% in all cases(P<0.01),the sensitivity was 79%,the specificity was 91%,the positive predictive value was 92%,and the negative predictive value was 77%.Conclusion The ultrasound mini-probe examination can provide high accuracy in the preoperative staging of colorectal carcinoma,especially in the extent of tumor invasion and metastasis of regional lymph node.The result may influence the therapyapproach.
