食管癌、癌旁和淋巴结组织中mdr-1、mrp表达及其预后关系 被引量:1

摘要 目的 探讨食管癌、癌旁及淋巴结组织中多药耐药基因 (mdr 1)和多药耐药相关蛋白基因 (mrp)表达的相互关系及其对患者预后的影响。方法 采用逆转录 聚合酶链反应 (RT PCR)技术 ,检测了 5 1例食管癌、癌旁及 49枚手术切除淋巴结组织的mdr 1和mrp基因表达。结果 食管癌组织中mdr 1、mrp和两基因共表达的阳性率与癌旁组织比较具有显著性差异 (P <0 0 1) ;转移淋巴结组织 (N1 2 )高于非转移淋巴结 (N0 ) (P <0 0 1) ;而癌组织mdr 1、mrp阴性的转移和非转移淋巴结组织未发现mdr 1和mrp基因表达。随访发现 ,食管癌组织mdr 1和 (或 )mrp阳性患者的一、二、三年生存率 (72 4%、42 1%和 14 3% )低于mdr 1、mrp阴性者(88 9% ,6 3 6 %和 6 0 % )。结论 检测癌组织及其淋巴结中的mdr 1、mrp基因表达可能对指导食管癌的术后化疗。 Objective To investigate the expression of mdr-1 and mrp g en es in esophageal cancer, surrounding noncancerous tissues and lymph nodes and it s relation with the prognosis of the disease. Methods 51 cases of esophageal cancer and surrounding noncancer ous tissues and 49 lymph nodes were obtained during operation. Expression of mdr -1 and mrp genes were measured by reverse transcription polymerase reactions (R T-PCR). Results Positive reates of mdr-1 and mrp in primary c ancer foci were 58.8% and 54.9%. While the positive rate of co-expression of th e two genes was 49%. These rates were significantly different with those of surr ounding non cancerous tissues (19.6%, 11.8% and 9.8% respectively) ( P <0 01 ). The postive rates of expression of mdr-1, mrp and co-expression of the two genes were 60.9%, 52.5% and 47.8% in metastatic lymph nodes and they were much h igher than those of non-metastatic lymph nodes (11.5% 7.7% and 7.7% respect ivel y) ( P <0 01). Postive rates of mdr-1 and mrp of metastatic lymph tissues w e re also significantly higher than those of non-metastatic lymph tissues. Data o f follow-up examination showed that 1-,2- and 3-year survival rates in patients with mdr-1 and/or mrp positive (82.4%, 47.8% and 23.1%) were much higher than those in patients with negative mdr-1 and mrp (92.3%, 62.5% and 60%).Conclusion The mdr-1, mrp gene expression in esophageal cancer tissues and metastatic lymph nodes may play an important role in directing post operative chemotherapy and predicting prognosis.
