摘要
目的 :观察急性脑梗塞患者血浆精氨酸加压素 (AVP)和内皮素 (ET)及一氧化氮 (NO)的动态变化 ,探求这些物质与脑梗塞严重程度的关系。方法 :用放射免疫法 (RIA)和 Green's法检测36例急性脑梗塞发病 48、1 6 8、480 h共 3次及 30例正常人 (作为正常值 ) AVP、ET、NO水平。结果 :脑梗塞后 48和 1 6 8h的 AVP、ET显著高于正常值 ,P<0 .0 0 1和 0 .0 1 ;而 480 h后与正常值接近 ,P>0 .0 5。 NO在脑梗塞后 48和 1 6 8h,非常显著低于正常值 ,P<0 .0 0 1和 0 .0 1 :而 480 h接近正常值 ,P>0 .0 5。脑梗塞大梗塞灶组 AVP、ET显著高于小梗塞灶组 ,P<0 .0 1 ;而 NO大梗塞灶组显著低于小梗塞灶组 ,P<0 .0 1。结论 :AVP、ET、NO与急性脑梗塞发病后的时间和梗塞范围有密切的相关性 。
Objective: To investigate the dynamic plasma levels of arginine vaso pression (AVP),endothelin (ET) and nitric oxide (NO) in patients with cerebral infarction.Methods: The plasma concentrations of AVP, ET and NO in 36 patients on 48 hrs,168 hrs,480 hrs after acute cerebral infarction and 30 normal controls (as a normal value) were measured by radioimmunoassay (RIA) and Green's methods.Results: The plasma AVP and ET levels of 36 patients on 48 hrs and 168 hrs after infarction were significantly higher than the normal value (P<0.001 and P<0.01).There were no significant differences between the levels of AVP and ET on 480 hrs and the normal value (P>0.05). The NO levels on 48hrs and on 168 hrs after infarction were significantly lower than the normal value (P<0.001 and P<0.01). There were no significant differences between the levels of NO on 480 hrs after infarction and the normal value(P>0.05). The AVP and ET levels in the group of large infarction zone were significantly higher than those in the group of small infarction zone (P<0. 01). The NO levels in the group of large infarction zone were significantly lower than those in the group of small infarction zone ( P <0.01).Conclusion: AVP, ET and NO are related closely to the progressing time and the infarction range of acute cerebral infarction, so they can be used as a clinical indicater to judge the severity of acute cerebral infarction.
