Cricohyoidoepiglottopexy:A report of 21 cases

目的 :探讨环舌会厌固定术的手术指征、功能保留及远期疗效。方法 :应用环舌会厌固定术治疗喉癌2 1例。结果 :一次拔管率 95 .2 4 % ,多在术后 2 8d内完成 ,无终生带管。全部恢复经口进食。发音恢复亦令人满意 ,无一例局部复发。结论 :环舌会厌固定术具有适应范围广、手术方法简单、长期肿瘤学与喉功能保留效果满意等特点 ,值得临床推广应用。

Objective:To assess the indications, function preserving and long term results of cricohyoidoepiglottopexy (CHEP) in the treatment of laryngeal carcinoma. Method:Twenty one patients were underwent this procedure during the period from 1993 to 1999 in our department. There were 20 glottic cancers (7 T 1N 0M 0, 10 T 2N 0M 0, 3 T 3N 0M 0) and 1 supraglottic type (T 2N 1M 0) according to the 1997 UICC system. In these cases, 15 were the aged over 60 years. Result:First decannulation rate was 95.24 %, and majority completed in 28 days post operation. No patient was with cannula all his life. All cases could finally take food by mouth without aspiration. Speech was also satisfactory. The overall 3 year and 5 year survival rates were 87.50 % and 75.00 % respectively. No patient experienced local recurrence. Conclusion:This procedure is presented as a useful alternative to partial vertical laryngectomy and total laryngectomy in select cases of laryngeal carcinoma.
